How do you say this in Spanish? Its for an essay and I don't want to literally translate it word by word because I know that doesn't always work. Thanks!

In English, when a noun is used as an adjective, it is usually used in singular form, so the correct English is multiple personality disorder. The Spanish is trastorno de personalidad múltiple.
