Intake process

I am looking for the correct phrase to use for "intake process," specifically
to describe what happens when a person seeks treatment for substance abuse.
In the U.S., the first step is the intake process, when the clinic or treatment agency
asks the patient a series of questions to determine the severity of their addiction.
