Same in Argentina. We use both but ¿cómo te llamas? is preferred.

¿Cómo se llama?
¿Cuál es su nombre'

Same in Spain, and in most countries, as far as I know.
"¿Cuál es tu nombre'" would be fine, maybe, if there is a list and someone is checking someone's name against it.
