You know the difference based on context. There are lots of words that sound the same in English, or that sound and are spelled the same, and we distinguish these based on context as well.

For example: bank of a river, bank where you keep your money; mail person, male person; etc.

Because "El tiempo" Time is "La hora" (¿Que hora es?) and the weather is "El tiempo" (¿Que tiempo hace en Madrid?)

You can use "El tiempo" for the time too, for example "Hace tiempo que no voy a casa de mi abuela" "Ha pasado mucho tiempo desde que nos vimos" but not for the hours.

The only way I know to tell whether tiempo means time or weather is by the rest of the words in the sentence. I don't know if I explained this as well as I could have but, that is the way it came out.
