Because we are talking about an unreality, a hope. Although is not required, in this case it sounds much better. If you want to use the future, you can say "...esperanza en que mejorarán las cosas" - cogumela, Oct 1, 2010

I think it's literal, gato.
Perder la esperanza.
And....I believe your example sentence would translate like this, but wait for confirmation.
Ha perdido toda la esperanza de que mejoraran las cosas.
