¿Cómo te llamas?

With these other options:

a. ¿Cómo te llamas? (informal) (singular) Hi! Nice to meet you. What is your name? ¡Hola! Mucho gusto. ¿Cómo te llamas?

b. ¿Cuál es tu nombre? (informal) (singular) I know you told me but I already forgot. What is your name? Yo sé que me lo dijiste pero ya lo olvidé. ¿Cuál es tu nombre?

c. ¿Cómo se llama? (formal) (singular) Welcome to our hotel, sir. What is your name?Bienvenido a nuestro hotel, señor. ¿Cómo se llama?

d. ¿Cuál es su nombre? (formal) (singular) I believe a package arrived for me. - Ok, what is your name?Creo que me llegó un paquete. Ok, ¿cuál es su nombre?

The form using llamar is the more common, natural way to ask.
