Thank you for adding some comedy to the Q&A forum. You are over-thinking this, and way too concerned about getting perfect grades. I'm a teacher and graduated at the top of my high school and while yes academic success is beneficial, seeking perfection only leads to stress-related illness. Okay, now that I have over-philosophized your question...
What's the common question your teacher usually uses to ask you your name? ¿Cómo te llamas? Right?
Now, if you look up the word "name" in a Spanish-English dictionary you'll get the word nombre, and even though most Spanish speakers would never use the phrase, lots of English speakers like to literally translate the English question "What is your name?" as ¿Qué/Cuál es tu nombre?"
Now if you want to go for extra credit why not try pulling some less common phrases from the super awesome Span¡shD!ct entry for "name"?
Seriously, you should check it out.
