'Tú' is a informal word for 'you', and 'usted' is a formal word for 'you'

(Formal and informal words are used while speaking, informal words are to be used with people you know and friends or family, formal words are to be used with people that are of higher rank of you, strangers, teachers, and people you look up to.)

¿Cómo se llamas? means "Whats you name?"

¿y tú? means "And you?"

¿Cómo está Usted? means "How are you?"

The first question should be either be ¿Cómo te llamas? familiar form or ¿Cómo se llama? usted form.
