I can help with some of this, but I don't know the correct or official translation for Social Security Disability Benefits.

Disabled? ¿deshabilitado?
Can't work? ¿No puedes trabajar?
Denied SSD benefits? ¿Se negó prestación de SSD?

Representantes de deshabilitados
No fee unless we win your case. No cuota a menos que venzamos tu caso.

I'm pretty sure of the last verb, but not certain. I think the subjunctive fits here. Maybe Lazarus, Heidita or another of the real experts can clarify.
