El término testamento vital, documento de voluntades anticipadas -DVA- o de instrucciones previas se refiere al documento escrito por el que un ciudadano manifiesta anticipadamente su voluntad

I've seen "testamento vital" in a couple of articles, but that just sounds so weird.
Just guessing, "testamento en vida" sounds better to me...but still....
