Your book is right. Pisacaballo's example is different (it is not reflexive). The verb "olvidar" can be used in three different ways:
alguien (se) olvida algo - (Me) olvidé el libro. (Te) olvidaste el libro. (Se) olvidó el libro.
alguien se olvida de algo - Me olvidé del libro. Te olvidaste del libro. Se olvidó del libro.
a alguien se le olvida algo - Se me olvidó el libro. Se te olvidó el libro. Se te olvidaron losl libros.
The pronoun in brackets is optional.

My book might be wrong. One example of reflexive use is:
(forget) !Caramba! Se me olvidó mi libro de ingles!
