How would someone hearing the Spanish sentence know that it is a question rather than a statement?

By the intonation , the same as in English it usually rises at the end , tho be careful in English there is a fad to speak like that by some people , which is annoying. - ray76, Oct 9, 2017

Questions that can be answered by a "yes" or "no" can be stated as Ray has said.

Example. ¿Rosa canta en el coro de su iglesia? Raising your voice at the end will get the idea that it's a question.

You can't go wrong by putting the verb FIRST. ¿Canta Rosa en el coro de su iglesia?

There are other questions that are obvious because they start with a question word.

¿Dónde está Pablo?

¿Cuándo es la fiesta?

¿Cómo se llama el profesor de álgebra? and so forth. These questions, however, are said by dropping the vocal pitch slightly at the end.
