I'm trying to say:

If he does not get better, when do you want us to call you again?

¿Si no se mejora, cuando quiere que le llamemos de nuevo?

Is the phrasing of this OK? Is my use of subjuctive in llamemos correct?

This is correct. Just missing the tilde on "qué".

I can see that the subject is a third person, but still I'd add a personal pronoun for clarity.

Not really necessary. Just my choice.

¿Si él no se mejora, cuando quiere qué le llamemos de nuevo?
