Apenas tembló cuando se cerró la puertezuela de un coche en la calle de al lado, ni cuando dos lechuzas volaron sobre su cabeza.

The suffix -zuela tends to be used for denigrating someone or somebody.
In this case the utterance is implying the door is clapped-out.
