When you say what did you eat for breakfast, do you say ¿Qué comes para desayuno? or ¿Que comes para el desayuno? Do you include the article or not, or does it not matter?

Your English variant is in past tense, so it should be: ¿Qué comiste / comió para el desayuno? In this case the article matters.

But a more natural way is: ¿Qué desayunaste / desayunó?
