Welcome to the forum Can you add your English and Spanish level to your profile?

I am not sure why you have bothered to ask this question. If you cannot remember the endings you can always use ir a plus the infinitive.

Well despite the above answers clearly not giving you A for effort. Here are the verb forms on a graphic. I am not going to tell you the answer as it is blindingly obvious.

That is a nice surprise - never was a Spanish verb so easy to learn!

This sounds like a homework question demanded by a methodologist rather than a teacher who is interested in teaching people a language. I suggest that you find the latter.
