The sentence should be ¿De donde es usted? and it is one of the first sentences that the teacher teach you when starting to learn a language:

Where are you from?

Another form of the same sentence is ¿de dónde eres?, which is the less formal way to say "where are you from" (the "tú" is left out because it is implied in the verb. It can also be "tú, ¿de dónde eres?" or "¿de dónde eres tú?"). All these forms are specifically used to find out a person's place of origin, not necessarily were he/she lives at the moment.
