lo que- means "what" in the sense that does not include the direct questioning way, ie in the sense that you could say "that which".

in some senses qué can replace it in the indirect question, again in a direct question you need qué.

No sé lo que quieres- I don't know what you want, I don't know that which you want.

you can also say: no sé qué quieres to mean the same (it must carry the accent to be correct here without lo, with lo there is no accent on que- it means "that" instead of "what?" without the accent- "lo que" is literally "it that").
