You can say / Puedes decir:
I really (truly) need help = De verdad, necesito ayuda
I really need help (I need a lot* of help)
= Necesito mucha ayuda
I really need help quickly (ie: in a hurry)
Necesito recibir ayuda lo más pronto posible / o lo antes posible.
