The meaning of this phrase?

Heard this line in a song I was just listening to:

"Sujeta bien hoy mi corazón. No dejes que caiga, no te alejes..."

I can sort of understand the parts with "no dejes que caiga" but the rest is a little confusing for me. I know sujetar means "to hold" in Spanish, which kind of make sense. xD

Today hold tight my heart. Don;t let (that) it fall, don't (you) move away (leave)..

I am not a native speaker and sometimes direct translations are difficult.

Note: I used tight for bien to make it sound better in English.
