La respuesta correcta es "a 33 year old woman".

She is 33 years old, but she is a 33 year old (woman).

Creo que sin un artículo es "years", con un artículo es "year".

No es posible decir "she is 33 years old woman", pero puedes decir "she is a 33 year old woman", "she is 33 years old", o "she is a 33 year old".

No sé por qué lo decimos de esta manera.
