Can you please convert the following question in the most appropriate form of usted!
Also can you tell me a hard and fast rule for "Usted" placement.
¿Vas a jugar al tenis la semana que viene?

Word order in Spanish is much more flexible than in English but in questions I would tend to follow the verb with usted (like you did in the third example).
¿Por qué lleva usted gafas de sol?
¿Cómo va usted al trabajo ?
¿Por qué está usted comprando una entrada?
For this one, it's fine but be aware that the word order gives a slightly different nuance.
¿Usted va a jugar al tenis la semana que viene?
The way you have it really stresses the 'you'. Like 'You are going to play tennis? You who has never played in your life!' - that kind of thing.
If you want it to be a normal question with no particular stress on the fact that it's that person who may or may not be going to play tennis then you'd probably put the usted after jugar.
And for this final one I'd put Ud. after the verb phrase ...

This is written correctly as a simple question without anything more than the action of going to the mountain. If you place tú in front of cuándo or after vas, it emphasizes "you" and with more emphasis of "you" when placed after vas.
You should also note that this sentence and your first sentences are congugated in familiar you (tú) , so you cannot use usted in these sentences without first changing the verb conjugations as Kiwi-Girl has done.

Yes, I'd definitely say that the most common order of questions that contain an interrogative (question word) would be ... Interrogative + verb + subject (if there is one).
