How do you say "Do you really care about me?"
I do care about you. ? "Me importas" So how do you say "Do you care about me?" Just add "?" and If you add "really", how do you say?

We don´t normally use importar about people, so for your senario it would sound more natural to say: ¿Me quieres de verdad?
Querer can mean love, like, care for, want.
¿Realmente te preocupas por mi?
¿Realmente te importo?
¿Realmente cuidas de mi?
¿Te tiene con cuidado lo que tenga que ver conmigo?
¿Realmente me quieres?
¿Me quieres de verdad?

I agree with JulianChivi, but I think "Do you really care about me?" can also be translated as ¿Te importo de verdad?
I say that because of my personal experience and understanding of the language (albeit fairly limited in the context of affairs of the heart) and the notion that "to care about someone" and "to be important to someone" are more or less the same thing. I think they are, but I never thought about it until now.
Do a Google search on "te importo" and you will see dozens of examples and images that imply that meaning.
Here are a few I found...
Si de verdad te importo, demostramelo! - If I'm really important to you, show me!
Nunca digas "Te Quiero", si no te importa - Never say I love you if I'm not important to you
¿Cuánto te importo del 1 al 10? - How are important am I to you on a scale of 1 to 10?
Is it the best translation? Maybe not ,but it's clear that it's (commonly?) used that way.
