This is courtesy of 'spanish examples website', The examples are for Spain.

The correct Spanish address format is:

Type of street – Street name – building number – floor number – door number Postal code – city province (optional, especially if the address is in a big city) Example Spanish address

C/ Santa Maria 45, 3º, 2ª 28012 – Madrid

The 3º means tercero (tercer piso) or third floor. The 2ª means segunda puerta or second door.

Speaking this address out loud you would say “Calle Santa Marie cuarenta y cinco – tercero, segunda”.

Hablando a alguien al que das tu dirección: Primero la calle, (Calle X) luego el numero (numero X) luego el piso si tiene (piso X) y luego la letra o el numero de puerta (Letra X puerta X)

C/Goya Nº8 piso 4º letra B
C/Atocha Nº19 piso 2º puerta 8

Dando tu dirección de correo: Calle, numero, piso, letra o puerta, código postal, municipio y provincia.

C/Princesa Nº6 piso 3º letra B CP:28943 Fuenlabrada (Madrid)

I think this answer may vary by country or even region. Here in La Paz we would say:

Numero 20, Calle Bolton, e/Revolución y Madero, Pueblo Nuevo, La Paz, BCS, Mexico.

e/ = entre or between.

This is how you would give directions to a delivery man or for mail. Less formal you would leave off the neighborhood, etc.
