Which mood?

I want to say "in case it was missed" in Spanish. But I'm not sure if I would I say "en caso de que fallara" or "en caso de que falló"?

Just looking at the tense and mood you would use the imperfect subjunctive. Your clues are ...
1) the english uses the past (was) so it's the past
and
2) the part with the verb 'it was missed' is not in fact a declaration that something was missed but is a possible outcome, so it can't be declared with the indicative and instead you need the subjunctive mood.
But what is the context because your example can mean 'in case it failed'.
