"Please fill in the following information..."

Question

Hello, everyone!

I am creating a simple "sign-up sheet" for persons to fill out who are interested in obtaining employment with a company (signing up does not guarantee employment, it just is a way for the company to contact people who are interested in being hired).

Could you please proofread what I have written in Spanish? I will write my Spanish translation guesses below the English that I am trying to translate.

with my limited Spanish, I see at least two things that can be fixed. For the "Please fill out the following information if you are interested in employment with our company," you should say Por favor and for the phone number, I think you would say Número de teléfono

To fill in a form: llenar is the verb normally used, I think.
