What's the difference between lleva puesto and just lleva? I have a story that uses lleva puesto but I don't see why you'd say that when you could just say lleva. Is there a difference?

I'll take a shot.

Llevar can mean to wear or carry along with many other things, but these two are important here. So solely to say llevar can mean wearing or carrying.

Ella lleva guantes. Is she wearing them or carrying them? I can't tell. But if you say: Ella lleva puesto guantes. Now it is clear that she is wearing them, i.e. has them in place.

Please correct me if I am wrong.
