Llevar a cabo implies that there is someone doing the action, therefore it means "to carry out."
An example from the SpanishDict dictionary:
Estamos llevando a cabo un proyecto en colaboración con la universidad.
We are carrying out a joint project with the university.
Llevarse a cabo is a passive verb, meaning "to take place."
An example from the dictionary:
En esta piscina se llevarán a cabo las pruebas de natación.
The swimming events will take place in this pool.
