I'm told by friends that in Cuba, almost everyone should be addressed as "Tu", whereas some other countries use "Usted" almost all the time. Does anyone have a list of country-by-country guidelines or have helpful hints for when to use the these different forms?

it is a good question (and subject title). I am no expert but I believe in some countries, such as Columbia, the formal address is widely used. It is my practice to use the formal usted form unless the informal tu is clearly indicated. If you were addressing someone more formally than necessary, they would usually let you know.
