What would be the correct grammar to say, "have I been blind, lost, etc.?" if one couldn't hear intonation and had to read it? Would the pronoun follow the verb yet be before the adverb, e.g., ¿He estado yo ciego, perdido, etc.?" or is it placed after the entire clause?

The personal pronoun is not necessary unless you want to emphasize yo.
If you choose to use it, it could go after estado just as you have written it, but it has more emphasis at the very end of the phrase: ¿He estado ciego/perdido yo?
Context and nuance will determine where and if to use this pronoun.
