¿Tienes dolor? If you are asking a friend or
¿Tiene dolor? If you are asking an older person. A doctor would use this form
The person could respond: Tengo dolor de cabeza = I have a headache
or Me duele la cabeza = I have a headache
Me duela la pierna derecha =My right leg hurts

you can say 'tiene dolor' or 'duele' which is a conjugation of 'doler' which means to hurt. Either is how I would say it.
