Does 'Lo' stand for the person we're saying we're sorry to? or does it stand for the thing we're sorry about? What does it exactly point to?

It's the thing that we are sorry about or regret.

Lo (it) siento (I feel).

Lo is a masculine direct object pronoun as well as a neuter; "it" is an abstract object, in a sense.
