Question On Subjunctive

Imagina un mundo en el que la medicina estuviera orientada hacia la salud en vez de hacia la enfermedad. Donde los doctores confiaran en la capacidad natural de curación del cuerpo humano y pusieran más énfasis en la prevención que en el tratamiento. En este mundo los pacientes y los doctores serían socios buscando un mismo objetivo. Andrew Weil, MD

Shouldn't this be?

Imagina un mundo en el que la medicina este orientada hacia la salud en vez de hacia la enfermedad.

And why all the use of subjunctive in the following sentences?

It's because it's a sentence that translates : Imagine a word in which medicine were.. A world that doesn't exist, hence it's all in the past subjunctive. In which the doctors would have/ were to have the conficence of teh patients, etc.. It's hypothetical.
