I can't seem to find a clear definition of this word.

Here's some example sentences I have found for it.

Es mi carnalillo de 12 años.

and

Que vas a llevar Carnalillo?

Here is the answer from my amiga boliviana directly from her email:

"LOL! Carnalillo comes from the word “ carnal.” It means “friend”. They added “illo” to it. It is mostly used in Mexico. Is the same as saying amiguito."

Carnal means carnal, by blood (in relationships) and buddy, pal (in Mexico).

The answer makes perfect sense, but I would've expected to see carnalito. Although a search in Google reveals more than a few people using canalillo.

Good question. Are you sure on spelling? Or just rare slang maybe? I struck out on both WordReference and Linguee, which almost never happens.
