sometimes they ask this way:"¿Qué tiempo tiene su bebé?" and sometimes "¿cuantos años tiene su bebé?" I want to know is there any difference?which of these is more formal?

"¿Cuántos años tiene .....?" is the more common way to ask "How old is ....?"

If someone asks you that question your response would be "Tengo xxxx años."

I would guess that depends more on the age of the baby. If the baby is young enough to be described in days or months it will be asked one way, or if it's old enough to look as if it is a year or two old the question with years would be more appropriate.

¿Cuánto /o qué tiempo tiene el niño? How old is the baby?
