What do the words "hace", and "hacía" mean, as in "hace una vez...", or "hacía una vez..."? Futhermore, are there any other forms of this phrase besides "hace...", and "hacía..."?

These are forms of the verb "hacer", which usually means "to make" or "to do". It can be used in various expressions that alter this, such as time references,in which case it can be interpreted as "ago".

Erase una vez / había una vez ..... Once upon a time

"Hace / había una vez" is a supporting clause and without context makes no sense.

Hace una vez means to do something once. Example:

Quien te la hace una vez te la vuelve a hacer.

Whoever screws you over once is bound to do it again.
