Must a direct object pronoun always be attached to the end of an infinitive, when it is placed after the infinitive? (I know it can come before the verb, as well.) As in this sentence: "Recibes una cheque por hacer lo de arriba." Does "lo" have to be attached to "hacer" in this case?

In this case, no because "lo de arriba" is a separate clause.

Without the clause "de arriba," then yes, the pronoun must be attached, ie: "Recibes un cheque por hacerlo" or "....... por haberlo hecho."
