"La Misma Luna" Question

In this clip of "La Misma Luna" from 2:35-2:42, the woman known as Doña Carmen tells the two younger people to call her "Doña Carmen," not just Carmen. She adds a reason, but I could not understand what she said. Can anyone explain why she insists on being called "Doña Carmen"?

She's working as a coyote and helping those men get to the U.S.

Muchas gracias!

She says "Doña Carmen, que mi trabajo me ha costado".

Doña Carmen, a title that has taken me time and effort.
