help with the hotel vocabulary

I have just found out that I will be working at a recepcion desk in the hotel and I know that in this type of places some popular sentences/ phrases are used very often. Could you please check them if they are used correctly (both in Spanish and English) because I don't want to be ridiculed at the very beginning? Here they are:

Do you have a reservation? ¿Tienen Ustedes (Tiene Usted) la reservación?

For what name? ¿A qué nombre?

May I have your name please? Podria Usted decirme su nombre, por favor.

Could you please fulfill this registration form in capital letters ¿Pueden Ustedes rellenar estos formularios en mayúsculas?

May I have your credit card? Usted podria darme su tarjeta de crédito, por favor.

Your room number is ... There are two keys - one is for the ....and the second for the....Breakfast, included in the price, is served every day between 7 and 10 at our restaurant which is straight and to the right.

Su habitación tiene el número... Aquí están dos llaves- el primer es para...y el segundo para....El desayuno, incluido en la precia, está servida entre las 7 y las 10 de la mañana en nuestra restaurante ( todo recto y a la derecha)

I´m still learning, but here are a couple attempts.

Could you please fulfill fill in this registration form in capital letters. This is probably a typo since "rellenar" can mean "to fill in".

There are two keys.....Aquí están Hay dos llaves
