responding to customers in a restaurant

Question

If a customer makes a request, how should the waiter respond to that customer, if the customer is a doctor? Example: Dra. González: La chuleta de cerdo, por favor. How would the waiter respond / address that customer?

Not too sure but something like,

Sí señor, poco, medio o bien hecho.

Muy bien. A la orden. Con gusto.

I've never worked as a waiter or doctor, but I imagine these would be well received.
