"Completed" - Another democracy en español question

How would I translate "a completed form?" As in, "Attach a completed form to your request."

I want to use "un formulario completado," but one of my native Spanish speakers insists this doesn't look right.

She says it is, "un forumlario llenado." First of all, I thought, when referring to filling out forms, the correct word is rellenar, not llenar.

Second of all, is completado(a) a word I false cognate? Sometimes I accidentallly use those. I also accidentally make up words that seem like they should be real, but are not. Is this one of those cases?

"Las solicitudes de registro de nacimiento deben de incluir este formulario debidamente/completamente lleno/diligenciado.

I think it should be "llenado". When you say "completado", you might be conveying the idea of a complete form, or a completed form. When you say this, you might be confusing people. They might wonder if all the pages of the form that they are attaching are present or included (which is a proper concern). But since you are trying to get them to make sure that every blank is filled in, for best communication, I think you should say "un formulario (spelling corrected in your 2nd version) llenado".

So I would say "Favor de fijar un formulario llenado."

I must warn you that I am not a native speaker of Spanish. There may be a more natural way to say it. On the other hand, I don't see how this phasing could be misunderstood by any Spanish speaker. And I think that is your goal....good communication. - Esteban3304, May 23, 2012
Another warning... if you are requesting some other kind of "birth record" that is somehow not referred to as a birth certificate, then replace that term with the correct one in Spanish. - Esteban3304, May 23, 2012
