Cannot add items to Flashcard

I cannot add new verbs to a personal existing Flashcard category. It currently contains the min 5 words, but when I click the 'yellow star' to add a new word, an error appears saying I need to create a Flashcard series. I've done the latter but the error always appears, any ideas?

Maybe its worth just rewriting the flashcards again in a new set and see if the problem persists

Click on the red feedback tab on the right side. Perhaps it is a software issue that the site owners can either fix or offer suggestions for.
