Aside from region, it also depends on the situation.

In general, you would say "qué pena (me da)" if you are feeling slightly embarrassed or guilty about something you have done or caused to happened. "Qué lastima" is "what a shame / pity" no matter who has caused the situation.

This depends on where you are.

In Colombia they are very different, qué pena means sorry, as in 'I'm sorry'.
