I'm looking for the phrase "sign-up sheet", as in - to sign up to bring food to a picnic. The translator gives me three choices: hoja de registro/incripción/signo.
I'm leaning towards 'signo' because I'm not sure that the other two have the same nuance. Any thoughts?

Formulario de inscripción, hoja de inscripción, hoja de registro.
- In your case, I think that "Hoja de inscripción" applies much better to the situation.
