If "What is your name?" is a question, you would ask, "¿Cómo se llama?" (formal) or "¿Cómo te llamas?" (informal). You answer this question with, "Me llamo ..." ("My name is..."). I can only add that in formal situations you could/may be asked for your name in this way :

¿Cuál es su nombre?

= What is your name?

Formal situations:

In a court of law in Spanish-speaking countries

In an official document

By a police officer

There may be other situations, too
