I believe one can say: el hospital del condado.
To say a specific county, one could say, "el hospital del Condado de Alameda" the hospital of Alameda County.
To say a specific hospital named after the county, you can capitalize the words hospital as in "el Hospital del Condado de Cook" Cook County Hospital.
