Informally: ¿Cuál es tu apellido?
More formal: ¿Cuál es su apellido?
the "ll" is pronounced like "y" in "yield"

In this instance Cuál is used in place of Qué as the English correlation would suggest.
Tip: To remember which to use -think of it like this: Out of all of the possibilities for someone's last name, your question is really asking "WHICH one belongs to them?" Therefore, ¿Cuál es su apellido? literally translates to Which is your last name?
