On the radio, I often hear an exchange like this: ¿Cómo te encuentras? Estoy muy, pero muy contenta estar aquí.
Is "muy, pero muy" a very common usage? Is it associated with a certain class or type of person?

It is fairly common, yes. It just is for emphasis, not necessarily any sort of person. It's like saying "very, very good." Muy, pero muy bien.
