I think Mizu wants to know if you start with your name, then middle name, then surname or something like that? Because I know some languages where you have to start with the surname.

Ah! Yes.

First name (de pila), second and sometimes more "first" names optional, generally no more than three "first" names, father's surname (paternal), mother's surname (paternal)

That's the way all of the Spanish speaking countries name their people.

What are you really trying to find out?

The way you write it in English is perfectly adequate. A tip, don't use your middle name if you have one, it can lead to confusion.

Why so many flippant answers? It's a valid question.

First name, father's last name, mother's family's last name.

If your name is Juan, your father's last name is Sanchez, and your mother's family's last name is Garcia, then your full name would traditionally be written Juan Sanchez Garcia.

This causes endless problems with the post office and other government offices here in Southern California.
