¿Han salido ya?

Yes, they have already left. -- Sí, ya han salido.

Have they left already? -- ¿Han salido ya?

I just want to be sure my placement of "ya" is correct in both a sentence or a question. Gracias!

Both of the placements work. with questions that ya should go there, and ya typically goes in front in statements. So, yes, you're right there.

Seeing that nothing should go between the helping verb & the past participle I think ya can go at the very beginning or the very end. I'm used to seeing it at the beginning though. eg. Ya voy. Ya nada me asombra.
